What’s in it
1. Services Provided
- The Provider agrees to deliver the services described to the Client, as agreed from time to time, in accordance with applicable professional standards.
2. Fees and Payment
- Fees are as set out in the Provider's current price list and are payable at the time of booking or as otherwise agreed. Invoices unpaid after 14 days may incur a late payment charge and may result in suspension of services.
3. Cancellation Policy
- The Client agrees to give at least 24 hours' notice to cancel or reschedule an appointment. Late cancellations or missed appointments may be charged in full.
4. Confidentiality and Data Protection
- Personal information is kept confidential and processed under the UK GDPR and Data Protection Act 2018, and shared only with consent or as required by law.
5. Limitation of Liability
- The Provider's liability for any claim arising from this agreement is limited to the fees paid for the services in question, except where liability cannot be limited by law.
6. Termination
- Either party may terminate this agreement at any time with written notice. Fees for services already delivered remain payable.
7. Governing Law
- This agreement is governed by and construed in accordance with the laws of England and Wales, and the parties submit to the exclusive jurisdiction of its courts.

Use a consent form before treatment starts, and again when what you are doing changes, so the client’s agreement is recorded in writing alongside their notes.
